I need to understand your invention in order to advise you about it, perform a patent search, or give you a quote for patenting.  You should write a description or summary and make one or more sketch(es) showing the invention.  Your description should include a list of the features you consider novel over prior devices, and the advantages of these features.  You should then obtain signatures of two witnesses on your description and sketch(es) as described in my General Information page. You can then mail or fax this form to me with your description and sketch(es).

  1. AGENT is a currently registered U.S. Patent Agent in good standing with the U.S. Patent Office.
  2. All materials provided by the above INVENTOR(S) to describe the above invention to AGENT are CONFIDENTIAL MATERIALS, unless a disclosure of the invention has already been made public.
  3. AGENT agrees to accept these CONFIDENTIAL MATERIALS under the following conditions:
  4. These CONFIDENTIAL MATERIALS shall be used by AGENT solely for purposes requested by the INVENTOR, such as a patent search, patenting, and drawings.
  5. AGENT agrees not to reveal these CONFIDENTIAL MATERIALS to any parties except as authorized by the INVENTOR.  Agent will provide these CONFIDENTIAL MATERIALS to a search firm if a full patent search is requested by the INVENTOR.
  6. AGENT shall exercise a high degree of care to safeguard these CONFIDENTIAL MATERIALS from access by unauthorized persons.
  7. AGENT shall only copy the CONFIDENTIAL MATERIALS as needed for the purposes above, and AGENT shall return the confidential materials to INVENTOR at any time upon request by INVENTOR.
  8. These terms shall not apply to any information which AGENT can document becomes part of the general public knowledge through no fault of AGENT, such as after publication of a disclosure of this invention by a patent office.
